EBAN 3D Collaborative - Defeating Diabetes Disparities
The second collaborative, titled EBAN 3D™ began in May 2013 and will end in June 2014. Each clinic team completes a project charter that details the scope of their project to improve diabetes outcomes in African American or African born populations.

- understand diabetes support systems available outside the care system
- A yearlong learning collaborative – May 2013 to June 2014
- Bi-monthly learning sessions through out the year. All teams attend.
- Each team meets individually between learning sessions
- Large scale initiative involving community partners, patients and health professionals working together on clinic teams to improve diabetes outcomes and community health
- We have 30 patients and community partners and 35 health professionals who are part of the collaborative
- Each team is given diabetes baseline data by focus population broken out by each of the five optimal diabetes components.
- Five teams will identify culturally-specific best practices to improve A1c, blood pressure and LDL in African American and African populations
- Teams will learn from each other to:
- connect the health care system with existing community assets and resources
- understand diabetes support systems available outside the care system
- Teams will apply quality improvement principles and measures to generate and test solutions to transform care delivery
- A collaborative effort that reaches across our system of care and into the community
- Teams will propose structural changes to systems and processes that will generate spread and value across HealthPartners Family of Care and the community
