Diabetes Education: Riverside, Midway and CIH Clinics Team

FOCUS: Our team is focusing on improving diabetes health outcomes through education for Amharic and Oromo populations.

OBJECTIVE:  How can we improve Diabetes care by Ethiopian patients who get their care at HealthPartners Riverside is through a series of three group classes over a period of six months? In order to close the gap in diabetes measures for HealthPartners Riverside patients this is our plan.

HOW WILL OUR TEAM MEASURE OUR SUCCESS: We will be giving out questionaires and surveys before and after each group class which will ask specific behavior change measures. We will enter and graph these behaviour changes to check our percentage of outcome.

GOAL: By December 2011, at least 80% of the class participants who have a Hgb A1C of equal to or greater than to 8.0% will show a decrease in the Hgb A1C by 0.5%.
