Pediatric Immunizations Team: Riverside and Brooklyn Center Clinics


FOCUS
: Increase the number of immunization rates at Riverside and Brooklyn Center clinic, for children up to 24 months and close the gap between children of color and white children.

OBJECTIVE: How can we increase 18-24 months with primary care visit immunization rate? How can we increase 24 months up-to -date immunization rates?

HOW WE WILL MEASURE OUR SUCCESS: Monthly 18-24 month with primary care visit and 24 month up-to-date immunization reported by race.  We will use Chi square yest to determine if there is significant difference between rates.

GOAL: To decrease the number of African American or Black children seen at the Brooklyn Center and Riverside Clinics who are not up to date with immunizations.

The Riverside Immunization Team is focusing on increasing pediatric immunization rates for children from East African Communities.
